Immigration Physicals

What is an immigration physical?

An immigration physical is a physical exam that you must schedule if want to become a legal United States citizen or permanent resident. 


The purpose of an immigration physical is to make sure that you don’t enter the country with any communicable diseases and that you have all your necessary vaccinations.


Once you’ve completed your immigration physical, your US Citizenship and Immigration Services (USCIS) authorized doctor completes Form I-693, also known as the Report of Medical Examination and Vaccination Record. 


You must submit this official government document to establish that, from the perspective of public health, you aren’t a threat to the US.

Do I need an immigration exam?

Typically, if you or your children are applying for permanent legal residency in the US, you need to schedule an immigration exam so that you can submit the Report of Medical Examination and Vaccination Record, or Form I-693.


Form I-693 is an official government document that establishes that, from a public health standpoint, you aren’t a threat to the US.

What happens during my immigration physical?

During your immigration physical, your Premier Family Medicine doctor first tests you for communicable diseases, such as syphilis and tuberculosis. Usually, you return two or three days after your tuberculosis test to get the results.


Next, they go over your chart to see if you need any vaccinations. Common required vaccinations include:

  • Diphtheria (DT)
  • Diphtheria, tetanus, and pertussis (DTaP)
  • Tetanus (Td)
  • Polio (OPV or IPV)
  • Measles, mumps, and rubella (MMR)
  • Haemophilus influenzae type b (Hib)
  • Hepatitis A
  • Hepatitis B
  • Varicella
  • Pneumococcal influenza
  • Rotavirus 
  • Meningococcal disease
  • Covid-19


To complete your visit, your doctor completes Form I-693 and delivers it to you in a sealed envelope. Take care to not open this envelope before you send it to USCIS because if it’s tampered with in any way, they won’t accept it.

Do I need to bring anything to my immigration physical?

Yes. When visiting Premier Family Medicine for your appointment, bring the following items with you to your appointment:

  • Form I-693
  • Government-issued photo identification
  • Immunization/vaccination record
  • Medical insurance card
  • Payment for your visit


Examples of acceptable forms of ID include: a driver’s license, current passport, or birth certificate with an English translation. 


Children under 14 must bring identification that lists their name, date of birth, place of birth, and parents’ full names.
